Our core goal is to provide 24/7 purposeful care and supervision to our residents. We provide assistance with bathing, toileting, grooming, medication, transportation, and any other aspect of daily living. We provide this care and supervision in a safe home like environment designed to sustain and reinforce a purposeful life for our residents.
We provide 24/7 long and short term adult residential care for traumatic brain injury survivors, with priority given to veteran TBI survivors. Our homes are located within Central California in Bakersfield, CA. Our residents are from age 18 and older. Our home is licensed by the California Department of Social Services Community Care Licensing. Our homes can accommodate 6 TBI residents each, with private room availability. Transportation is provided to regular outings and medical appointments. Nourishing meals and care are provided in a harmonious, family environment.
Arthritis, physical disabilities, and age-related conditions are some of the few circumstances that prevent patients from caring for themselves. Our caregivers assist you with daily tasks such as mobility, eating, exercising, and grooming.

Our Enhanced Care Management program is a CalAIM whole-person, community-based care management benefit for Medi-Cal members with the most complex medical and social needs. Each member is assigned a dedicated Lead Care Manager who meets them where they are — at home, in a shelter, at a doctor's office, or in the community — to coordinate all aspects of their care and help them achieve better health outcomes and quality of life.

We provide safe, short-term housing for up to six months for individuals who do not have a stable residence and have been recently discharged from a hospital, residential treatment facility, correctional facility, nursing facility, or recuperative care setting. Our program supports continued recovery by offering a stable environment while members work with their care team to secure long-term housing and connect to ongoing services.
Our recuperative care program serves individuals with unstable housing who no longer require hospitalization but still need time and support to heal from an injury or illness. We provide short-term residential care that includes housing, meals, ongoing health monitoring, and coordination of transportation to medical appointments — giving members a safe place to rest and recover.
We help individuals who are experiencing or at risk of homelessness find, apply for, and secure stable housing. Our team guides members through every step of the process — from identifying available housing options and completing applications to navigating eligibility requirements — so they can move into a safe home as quickly as possible.
Once housing is secured, we continue to support members in maintaining safe and stable tenancy. Our services include coordination with landlords to address issues, assistance with annual housing recertification, and connection to community resources to help prevent eviction and promote long-term housing stability.
We support members living in nursing facilities who are ready to transition to a less restrictive, community-based assisted living setting. Our program helps facilitate the move and ensures members receive the daily living support, medication oversight, and 24-hour care they need to thrive in a community environment while avoiding unnecessary institutionalization.
We assist members transitioning from a nursing facility to a private residence where they will be responsible for their own living expenses. Our program provides funding and support for essential set-up needs including security deposits, utility set-up fees, and medically necessary items such as air conditioners, heaters, or hospital beds to ensure a safe and successful transition home.
